To define a personal initial on the Client Card, head over to the Details-view, under the Name section.
Click on Set initials...
The following dialog will appear.
Click on the field and enter the initial you want.
Click anywhere in the empty space, and the changes will be saved automatically.

Note:
1. Ensure that you are either a Case Manager, a Client Manager, a User with 'Assistant'-role, or a User with 'Administrator'-rights set up to be able to make changes.
